Recognizing that love and attention can be powerful medicine, we encourage family and friends, including children and pets, to visit their loved ones. We recommend visiting between the hours of 8:00 a.m. until 8:00 p.m. This provides our residents with the time they need to prepare for their day and obtain the rest they need to participate in their treatment and daily activities.
Planned admissions are accepted 24 hours a day, 7 days a week, including holidays.

Our Rehabilitation Program will consist of an interdisciplinary team to evaluate each member’s unique situation and create an individualized plan of care to attain the greatest outcomes possible. During treatments, our therapists will work to improve independence, strength and confidence to transition our patients back to their prior living situation or the least restrictive environment upon discharge.
Long-Term Care assists those who may benefit from 24- hour nursing availability and assistance with meals, medications or other activities of daily living. The environment at Falcon Ridge is designed so that members may live comfortably, as a part of a community with a focus on health and wellness.

At Falcon Ridge, our services include 24-hour clinical care, licensed nursing care, comprehensive wound care, pulmonary management, post surgical stabilization, intravenous/enteral therapies, restorative nursing care, tracheostomy care, PICC lines, gastric tube care, NG tube care, TPN nutrition, diabetic program, respite care, hospice care, and physical, occupational, and speech therapy.
Falcon Ridge’s extensive therapy department is equipped to meet virtually any rehabilitation need. With the latest equipment in our 3,500 square foot therapy gym, a mobility garden, ADL kitchen, ADL bathroom, Transition room, and anti-gravity treadmill we can help those who require either short term or long term complex rehabilitation. Qualified licensed therapists provide comprehensive rehabilitation treatments in three primary disciplines of Physical, Occupational and Speech Therapy.

Falcon Ridge is committed to maximizing the functional capabilities of each member and achieving each person’s highest practical level of independence.
Therapy treatment is provided by our skilled clinicians in our spacious and fully equipped therapy center that is designed with the latest therapy assessment, monitoring, and treatment modalities.
Our team of clinicians specializes in understanding the individual needs of our members and how to best adjust the pace of rehabilitation to maximize success for each patient.
The team provides a full range of services including: mobility and endurance recovery, communication and cognitive augmentation, dysphasia management, orthotic fitting and training, pain management, activities of daily living and community living skills restoration.
We tailor our members’ rehabilitation to simulate a home like environment with our ADL Kitchen, Bathroom, and Laundry areas, as well as our Transition Suite and Mobility Garden.
Under the direction of an independent wound care physician, our licensed wound care nurses work seven (7) days a week for members requiring treatment for complex wounds, including but not limited to post-operative wound sites, diabetic and decubitus ulcers.
An important part of Falcon Ridge’s rehabilitation program is our Transition Room. Members can gain practice in skills such as cooking, bathing, and transferring in and out of bed, in a realistic home-like setting.
Our Transition Room comes complete with refrigerator, microwave, dining area, shower, and bathroom in a private room to help members regain confidence for independent living. This allows members to assess their capabilities to care for themselves and our team of therapists and nurses to identify any areas of concern in a home like environment prior to discharge. This increases confidence for a safe and successful return home.
Falcon Ridges’ menu follows a liberalized philosophy supported by the American Dietetic Association and the American Diabetes Association. Our Culinary Services Director will meet with you to discuss any specific restriction and /or preferences you may have.
The facility provides breakfast, lunch, dinner and snacks to our members. We encourage them to eat in the Bistro or Cafe in order to socialize with fellow members; however it is perfectly acceptable if the member prefers to eat in their room.
We offer a person-centered approach to our members dining experience. Members are served restaurant style and their selection is taken by our staff at the time of the meal. Our meals are prepared fresh daily and are designed to accommodate all members, including those with specialized dietary needs or modified texture diets.
Falcon Ridge Rehabilitation provides various recreational activities to meet each member’s spiritual, social, sensory, cognitive and physical needs. Activities are adjusted to accommodate each member’s level of ability. We provide individualized and group activities.
Our recreational program includes music, arts, crafts, games, religious services and outings. Members receive a monthly calendar of events and reminders so they can participate in events.

Our professional staff, and the independent physicians and consultants at Falcon Ridge Rehabilitation are all committed to working with our members and families as partners to meet their individual health care, rehabilitation, and coordination needs. As care givers, we want to know our members as individuals, taking care of their health needs, but also considering the person as a whole.
